I had sent her a message sayin we could hang out when she was in the city. She said it sounds good. After that I sent her another message but that was never replied. I know she actually is kinda busy, other friends of hers are also complaining she never has any time to answer them on facebook.
So the thing is, I've waited enough, its been two weeks since my last message.
What kinda message should I send her so she would remember the thing about our date, and actually want to meet me? I know there are a lot of talented guys in here, so it shouldn't be a problem for you to give advice on a message to get a first date. Other than that, I'll manage with whatever talent and education I have acquired. Just please give me some advice on a perfect working message.

Then it got put on the back burner and forgotten about. We're gonna' be optimistic here, because if it's simply a matter of her not liking you anymore and ignoring you, then you really don't have many options at this point.
You can try reiterating: "Hey Sara, I wanted to see if you were still interested in getting together sometime? I pm'd you last month but never heard back." Yes, it's needy, but again, your options are limited here. This type of response is also good because it lets her know that you have tried a couple times now to arrange the meeting she agreed upon and she isn't responding. It kind of puts her on the spot to respond and answer for herself.
Or you can try an attention getter. Don't make mention of previous attempts to contact her. Don't ask her out again. Just send her some exciting/random/funny message that you think will grab her attention and prompt her to respond. Something like: "Hey Blondie, just thought I'd give you a shout. I heard there were some bath salt zombies roaming in your neck of the woods so I got a little concerned.
