First of all a # is not a close it's just a bridge to another time when you can close. In my honest to god opinion a close is either a relationship or for some sex.
#1 Stop putting all this pressure on yourself. It sounds like you got the weight of the world on your shoulders and my guess is you aren't having a lot of fun out there. Remember to relax and have fun...things get easier when you do this. Don't care about the outcome so much!
#2 When things are going well asking for her # almost takes care of itself. You two are having a great time so it's easy to say, "We have so much to talk about. I got to go soon but give me your # so we can continue this later."
-Just hand her your phone and don't say a word. She'll know what you want.
"How can we continue this conversation?"
"Give me your #, I totally have to call you later."
Don't ask for the # just assume you are getting it. When you ask she has to say Yes or No.... which is 50/50. When you tell her to give you her #, you get a better response rate.
CHAT
