This brunette girl behind me goes "How about a macchiato?"

"Oh okay" I look back over to the lady behind the counter, "sure macchiato."

I was wondering what could I have done differently? I was thinking about asking for her number but I felt like it was too soon. I was thinking afterwards I could've said something like 'How do you want me to tell you how much I like or don't like this coffee?' and bring out my phone. Is that a bad? I didn't really get any IOIs. I always hear people saying you can get three IOIs within 2-3minutes but I don't quite understand how.

Anyways the "what's your number" part has to sound so casual and cool like it's old-hat to you. Practice it till you get it down. If you whip your phone out and talk all awkward she might have second thoughts. A guy who has value doesn't act all geeked-out over getting someone's number.

Does your posted convo look in any way interesting?

The most fundamental thing you are doing wrong is initiating whats called a "ping-pong" or resume/CV conversation with her. Asking questions alone to build attraction is a fail. You want to make more statements, hopefully funny, interesting, or intelligent statements that spring from how she answers the questions. The main thing your trying to do is to be interesting to make her attracted after you open.

So no you didn't mess up by not asking for her number from what you posted here as I doubt you generated any attraction. What you need to work on is practicing opening girls and creating interest.
