Every now and then I'll tease my gf really crudely and she'll get really defensive and tell me I'm being obnoxious and inconsiderate. Is ignoring her reaction best?

No. If she is actually offended (and not playfully fake-offended), learn to stop yourself the next time you make a bad joke. Legitimately offending your girlfriend isn't part of a relationship, and if you do offend her, you can always apologize.
