ok heres the deal, when im hanging out with friends and a friend of mine invites a girl then i have no problem talking to her what so ever, also when i am at a house party i have no problem talking with girls there also, but when im at like the gym or a club/bar its harder for me to approach girls there,

so any ideas???

The reason those approaches are easy is because your not putting yourself out. See, if your friend invites a girl over, you talk and she piss's off. You've lost nothing, right?
And at a house party, you know at least a few of the people there, and there not ALL strangers to you, so its easy, because your comfortable.

In the field however, when you see a super fit bird, and you know all eyes are on her all the time, your bottle go's abit, because if you fuck up, you know all eye's have seen you fuck up.

Here's the trick though. The thought that will carry you threw that fear. If you don't grow the ball's, and get up and talk to her, you will never get her. EVER. She will not notice you and think "bugger me, who's that bloke" then trek over to you and start rubbing you cock.

So, you've got to go, it aint easy, in fact its one of the hardest things to do in this world but if you let fear hold you back, in essence, your fucked mate.

And ah, you can always build up you game by opening lesser birds. Then when you go over to the fit ones, you'll want all those eyes on you, because you'll want every bugger to see you are making out with the girl they'd all sell there left nut's to even be seen with. And that's exactly what you'll get up to mate, bit a time and a bit of practice is all that's between any of us, and greatness.

Maybe the thought of what you do with yourself after you get rejected is screwing ypou up a bit bro, thats why a time constraint is such a good line, because you can get out of there any time just by repeating what you had to do.

I also used to have a bit of approach anxiety, how I got over it was to go to a bar or club by myself and play the game from the second I got it there. Walk over to the bar to get a drink, be sure to stand next to your target or her friend and open them up. give a time constraint then move on, tell them you might see them a little later. Make your way around the club or pub getting comfortable with the fact that you're by yourself and making new friends the good thing is that if everything goes bad, you can move on without anyone knowing. When you're out of there you can review your evening and maybe place a post on here and see what went wrong and what went right.
Field testing yourself is the best way to overcome smaller obstacles in my opinion.
