How you introduce it depends on the location and settings. If you've started talking, you may want to go outside for some air. Grab her hand or put your hand at the lower back of her. If a strand of hair has fallen before her face, you can remove it for her without stopping to talk/listen. Stuff like that are great for introducing kino. Another good way would probably be to touch her arm (if they're on the bar disk in front of her) as you exclaim something like "Did you know?" Followed by "Today I [something]" or "You really have [something]" etc.

It does not necessarily need to start on the arms. hands and lower backs are also good. or simply touching her while dancing.

I would not take an acceptance of kino as an IoI, but if she returns it: definitely.

At this point, unlikely. Try it, maybe it'll work, but don't be surprised if it doesn't. I'd say to focus on body language and frame in general for a bit, then revisit this.
