Definitely make a journal to track your progress.
Here's how my journal is constructed:

I choose between writing an "entry" and a "field report". If I choose to write an entry, it can be anything that's on my mind - game related. Sort of like a personal blog you could say.

So for example as title: Entry 1 - The Beginning (January 1st 2011).
And then I write anything that's on my mind. . . Maybe I figured something out that I wanna try in-field, for example.

And whenever I got out sarging, the title becomes something like this. . . : Field Report 1 - A messy situation (January 10th, 2011)

In the field report I explain the following:

* What I did that night (basically - what happened?)
* How I feel that it went

and then I conclude it with:

*What I did good
*What I can improve for next time

I reward myself for the good stuff I did - and I plan my next sarging with the improvements that I've written down.

It's always important that you analyze your night and you WRITE down what you need to improve. Equally, it's important to write down what you did GOOD.

Why do you need a layout? It's your journal.

You create it the way if suits you the best. Don't follow some weird ass layout which may or may not even fit with the way you write.

Do as I did. Either use it the way I'm writing, or come up with the most convenient way of writing yourself.

Way more fun that way too!

Don't just copy one's style. Remember - everything you do must fit your character.
As must your journal - because it's your most personal information detailed in it.

But Panda gave a solid lay-out pattern or help pattern. Look into it - adjust it if you want to or else just do your thing.

Good luck!
