An exception to the rule would I guess women who rest like 10 minutes in between sets, hardly break a sweat and spend most of their time messaging their friends and updating their FB status with "omg in the gym, getting hawt" lol. Have not seen many of those though but they exist heh.
Besides that, I never understood why people would go to the gym and then be distracted by starting to pick up girls . . . If you're at the gym, you're there for the one and only purpose of working out. It's not a place to pick up girls and shouldn't be treated that way either.
Years back, the gym used to be one of the best pick up places in town.
That's subsided somewhat, but the premise that it's somehow wrong doesn't apply.
If the girl gives IOIs and shows she's receptive to an approach, then you make it. Going so outwardly confrontational isn't clever, nor cool - it's creepy, and garners you a discussion with the manager.
I've never worried about being confronted by a manager of an establishment. I trust myself in social situations. But you can bet your ass if I'm at the gym and a lady is giving IOIs, I'll make conversation. I make conversation EVERYWHERE. It's who I am - and if I decide to run game, I will - I don't care if it is a gym...
