First of all, dinner & movie - bad idea. Dinner puts alot of stress on the interaction and forces you to come up with stuff to talk about on the spot, while during the movie, you're both sitting there doing nothing - what does that demonstrate about you? That you know how to put your phone on vibrate?
Since you're posting this in the newbie section, i can fairly assume that you have no idea what you're doing when keeping an extended conversation with a woman. The idea is to find an activity that you can both enjoy together such as ice skating, a dance lesson, or window shopping - an environment that gives you stuff to talk about and relate to your own life.
If you're determined to stick with the D&M date, no worries. The most important thing is to break the touch barrier early on. Don't make any big movements or linger too long. Let it be subtle. When walking her from the car to the movies/restaurant (whichever you're doing first), grab her hand (gently, calmly, and with a nice smile on your face) and lead her to the entrance.
Rebuild attraction with DHVs, show that you're not too cool for school and tell her an embarrassing story of yours as an adolescent - nothing gross or value-lowering, something you would even be comfortable telling her mom (e.g. your first kiss when you were a kid and make it sound cute).
During the movie, look over to her, grab her hand and interlock fingers and hold hands for ~30 sec.
Then use the fail safe - Mystery's Kiss Gambit.
-Whisper in her ear "Do you want to kiss me?"
A) If "yes" - kiss her
B) If "maybe"/"I don't know" - say "let's find out" - give her a peck on the cheek, then sit back and smile at her. Proceed with (C).
C) If "not here" say "I understand" - when the movie is over and you walk out of the movies lead her by her hand to the wall or an area near the entrance (somewhere semi-secluded) and kiss her.
D) If "no" - say "I didn't say you could, it just looked like you had something on your mind. then let go of her hand, smile at her and continue to watch the movie.
While kissing after bit, pull out early, look at her, smile and say "you're bad, i've gotta watch out for you" then go off and do something else (finish watching the movie or lead her to your car, depending on where you are)
