Help with Approach Anxiety

Hey everyone, I'm a highschool student and there's this girl in my summer school class that I really like. I think she might be interested because we have eye contact over and over again, but I'm too nervous to go talk to her. I have a severe case of approach anxiety and she's one of the prettiest girls I know. Other guys have started flirting with her, and I really want to open our first conversation. So my questions are:

- What're your thoughts on this?
- How do I maintain our eye contact?
- Should I smile when we look at each other?
- How do I approach her without being nervous?
- What can I talk about?
- Any tips on how to keep the conversation going without those awkward silences?
- How do I get close enough to get so we can have lunch together alone?

I only have about 3 weeks left of school and I really want to get as close as possible with this girl. I feel like I actually have a chance, but when I feel like I'm ready, something always stops me and I make an excuse on why I couldn't do it. I'm tired of excuses, I want to get it done.
