Calling a HB, how do I ask for the date?

Hey guys. I'm calling a HB tomorrow to try and set up a date. This is like the first one I've ever done so I'm new to this. What exactly should I say when I call her, like from A-Z I'd appreciate some tips.

(1) What do I tell her when I call
(2) How do I set up the date
(3) What place has gotten you the most success on a first date. (Where to take her)
