He definitely didn't deliver it right then. It's called tongue in cheek. It's supposed to be an obvious joke from the start based on the facial expression and then the laugh. Haven't had a creeped out target yet.
It could also depend on the maturity level of the girl. If you take this line to a 16-17yr old brat i can see her freaking out. However if you approach 23-24 yr olds the rate of success would be significantly higher i'd imagine.
Actually, it's the other way around. Idea of babies is cuter when it's not contemporary.

I honestly can't really picture this line working on sets. Here's why. What do you expect the girl to actually say in response? She won't really know what to say, which will then result in her feeling uncomfortable and she'll be more likely to blow you out. That's why openers should be constructed so that the girl is prompted to give you an expected response which you can play off of.
