Another problem of mine is that I fake myself out thinking that I'm not afraid to approach any girl, which is true, but I can't approach a girl in front of her friends, in front of my friends, or with anyone I know being around... Any idea how to fix these issues? It's like I think I'm not a very self conscious person but I really am, and I really do care what everyone else thinks about me and I don't believe for some reason that I can be "that guy"

As for the first, you might try to.. listen.. to what she's saying, the ton of the voice, her body language etc. I mean its something really simple, yet so many people dont listen attentively while talking. This way you would find a lot of stuff you can ask her about or comment on. Remember, the official reason you are talking to her is because you are social, not because you are trying to get her attention. Thus listen to what she's saying, and try to get it moving for the sake of getting to know her better, not to get laid.
As for the second. Well.. i guess you just have to put a smile on, stop being defensive and yet again look at it from the perspective of being a talkative friendly person. You can use a couple of openers on the group to get their attention. Its easier to get into a the group first and than get her out, than just approaching her directly.
I also have those problems, but the letter mentioned stuff helped me;) Hope it helped you too.
