Approach anxiety is there in all kinds of levels. You can reduce the level in a great way:

Get nice sunglasses, wear clothes that are 'hip', get nice shoes.
Fashion is very important, it can make a nerd to a stud with only a few extra clothes/layers.

If you want to eliminate approach anxiety, first read about the way they reject you. They are rejecting your approach, not yourself. They don't even know you so it's just about how they feel at that moment. If they haven't fucked any guy for a month and they are searching for a new boyfriend, you found the right girl. Of course, those girls aren't everywhere. If you approach 5 girls, I garantuee you: 1 girl will stop and talk to you.

So it doesn't matter AT ALL.
What you need to focus on is: APPROACHING NO MATTER WHAT.

The next time you go somewhere to pick up girls, say this to yourself: 'If I stay here looking like a homosexual I am not going to make progress in my life. I am not going to bang the chicks I want. I am going to just say something to that girl over there, like 'nice bag, where'd you get it?'.
Everytime you are in that state of not daring to approach, just remember the 3-second rule.
You just have to say anything!
