Is it normal for a girl to almost completely ignore you (after being overly flirty) when you fail to escalate and read IOIs?

If this is the case, could it also be that she's still interested and merely donning a different approach in order to ‘lure in the prey’?

Do girls easily forget previous attraction when the dude fails to capitalize? Or does it haunt them?

You cant generalize every situation, but personal experience if you don't capitalise on the opportunity when it presents itself then she can take it as a rejection and then if later you crawl back to her shes thinking wtf, you see girls always have guys approaching them, if your gone other opportunities come up.

as for your situation specifically I don't have enough details to make an accurate judgement on why she acted that way

They are still human, and like any human they have regrets...with some guys they regret it with some they could care less. Depends on the girl and your chemistry with her.

Yes, it's normal; generally it's better to escalate too quickly than not at all. The lack of proper escalation will at best get the girl confused as to what your intentions really are, the reaction depends on the girl though. A friend/wing of mine told me one girl got crazy mad at him when he failed to escalate.
